Dr. Shalom Fialkoff, DMD, MPH
Dr. Fialkoff believes strongly that healthcare is changing towards a collaborative, decision-making model. His goal is to diagnose your dental conditions and provide options to help. He devotes as much time as necessary to discuss all the different options, the pros and cons of each, and encourages patients to research those options before finalizing their plans.

Dr. David Lebowitz, DMD
Dr. David G. Lebowitz was born and raised on Long Island, New York. He received his undergraduate degree in psychology from the State University of New York in Binghamton and went on to study dentistry at the University of Pennsylvania in Philadelphia. Upon graduating from the University of Pennsylvania Dental School in 1979, Dr. Lebowitz was accepted into their post-doctoral residency program.

Dr. Jerry Sparks, DDS
Dr. Sparks had been practicing family and cosmetic dentistry in the Phoenix area for over 40 years. He is married with four grown children, three daughter-in-laws, one son-in-law and nine grandchildren.
